Unfortunately, surgery is performed because no other form of treatment worked and not because the clinical indications showed that the cause of pain could be resolved through surgery. Surgeries that fail at resolving neck and back pain are even termed “failed back surgery syndrome,” coined by those performing the surgeries to explain the continued or increased pain experienced after the surgery.
Tests such as X-rays, scans or blood tests may be advised in certain situations. This is mainly if there are symptoms, or signs during a doctor’s examination, to suggest that there may be a serious underlying cause for the back pain.

You’re not paranoid if they really are after you! Only about 1% of back pain has a nasty cause,17 and only a few of those are really scary. But it happens. Andy Whitfield, star of Spartacus, thought he just had back pain from his intense gladitorial training. In fact, he had a tumour. It killed him in 2011.
Upper back pain may also be caused by arthritis. It is true that most arthritis occur in the knees and other joints. But it can also cause upper back pain as arthritis is any inflammation of the joints and the spine is composed of a number of vertebrae attached at joints.
Try taking one 250-milligram capsule of valerian four times a day. Some scientists claim that this herb’s active ingredient interacts with receptors in the brain to cause a sedating effect. Although sedatives are not generally recommended, valerian is much milder than any pharmaceutical product. (Valerian can also be made into a tea, but the smell is so strong-resembling overused gym socks-that capsules are vastly preferable.)
Break out that bag of frozen peas (or an ice pack, if you want to get fancy) for the first 48 hours after the pain sets in, and put it to use for 20 minutes a session, several sessions per day. After those two days are behind you, switch to 20-minute intervals with a heating pad. Localized cooling shuts down capillaries and reduces blood flow to the area, which helps ease the swelling, says Lisa DeStefano, an associate professor at Michigan State University College of Osteopathic Medicine in East Lansing. Cold also thwarts your nerves’ ability to conduct pain signals. Heat, on the other hand, loosens tight muscles and increases circulation, bringing extra oxygen to the rescue.
Back pain usually affects the lower back. It can be short-term, lasting a few days or weeks, or for some people, a long-term condition. Most people will have some form of back pain at some stage in their lives.
Given the prevalence of back pain, you would think we’d have treatment for it down pat. Not even close. “The challenge is that you can’t see injuries to tendons, ligaments, and muscles the way you can bone fractures and herniated disks,” says Jeffrey Katz, MD, a professor of medicine and orthopedic surgery at Brigham and Women’s Hospital in Boston and the author of Heal Your Aching Back. Spinal manipulation, for example, is controversial — some docs say it does more harm than good — but it’s the only remedy that got me off the couch and back on my bike. “Chiropractic care is not without concerns; then again, neither is traditional medicine, particularly when you’re dealing with a problem like back pain, which has no easy, one-size-fits-all fix,” says Mark Moyad, MD, a FITNESS advisory board member and the director of preventive and alternative medicine at the University of Michigan Medical Center.
Ice it. Applying an ice pack to the painful area within 24 hours of an injury can help keep inflammation to a minimum and ease discomfort by decreasing the ability of nerves to send pain signals to the brain. Place ice cubes in a plastic bag, then apply bag on top of a thin towel that has been placed on the skin. Leave the ice pack on for 20 minutes, take it off for 30 minutes, then replace it for another 20 minutes.
Modification of a patient’s physiological response may be necessary. It is widely accepted that stress and tension can have negative effects on a patient’s overall health and well-being. Biofeedback is one form of therapy that has been shown to be beneficial for patients suffering with low back pain by helping them to reduce pain, stress, and tension. This therapy is a non-invasive, non-pharmacological treatment that helps patients recognize their symptoms and teaches them ways to modify their body’s natural response.

Although middle back pain is sometimes considered synonymous with thoracic back pain, it can generally be described as pain that occurs above the lumbar region of the spine but below the rib cage. In cases of middle back pain, the symptoms can be vague and difficult to diagnose, which can be especially frustrating in chronic cases.
Antidepressants may be effective for treating chronic pain associated with symptoms of depression, but they have a risk of side effects. Although the antiseizure drugs gabapentin and carbamazepine are sometimes used for chronic low back pain and may relieve sciatic pain, there is insufficient evidence to support their use. Systemic oral steroids have not been shown to be useful in low back pain. Facet joint injections and steroid injections into the discs have not been found to be effective in those with persistent, non-radiating pain; however, they may be considered for those with persistent sciatic pain. Epidural corticosteroid injections provide a slight and questionable short-term improvement in those with sciatica but are of no long term benefit. There are also concerns of potential side effects.

A pharmacological approach is often taken for patients suffering with lumbar spine pain. There are a number of drug therapies that are available to these patients, which may be used alone or in combination with other treatment methods. One study investigated the management of lower back pain patients who were seen in a primary care setting. The results showed that the majority (69%) of the patients were prescribed an anti-inflammatory drug (NSAID), followed by muscle relaxants (35%), narcotics (12%), and acetaminophen (4%).
Examples of possible treatments and physiotherapeutic methods targeting low back pain are also presented. Physiotherapeutic procedures used to treat low back pain include exercises in safe positions, balance exercises, manual therapy, massage and physical measures.
Spondylolisthesis gets its unusual name from the Latin word for “slipped vertebral body.” When one of your lumbar (lower back) vertebrae moves forward over the one below it, you have a condition called degenerative spondylolisthesis. The movement of the vertebra can irritate the nearby spinal nerve root, causing pain in the lower back and legs.
If back pain doesn’t respond to exercise, a medical practitioner will likely recommend a course of conservative treatment methods, including physical therapy and chiropractic treatment. Research has shown that the majority of patients (90%) who take part in conservative treatment report a significant reduction in their low back pain symptoms.
When other therapies fail, surgery may be considered an option to relieve pain caused by serious musculoskeletal injuries or nerve compression. It may be months following surgery before the patient is fully healed, and he or she may suffer permanent loss of flexibility.

Workers who experience acute low back pain as a result of a work injury may be asked by their employers to have x-rays. As in other cases, testing is not indicated unless red flags are present. An employer’s concern about legal liability is not a medical indication and should not be used to justify medical testing when it is not indicated. There should be no legal reason for encouraging people to have tests which a health care provider determines are not indicated.
Relax. Much back pain is the result of muscles made tight by emotional tension. Learn and practice a relaxation technique, such as meditation, or try a deep-breathing exercise, such as closing your eyes, breathing slowly and deeply, and counting backward from 100.

Also, sleep in the most naturally comfortable position on whatever is the most comfortable surface. Advice given in the past used to be to sleep on a firm mattress. However, there is no evidence to say that a firm mattress is better than any other type of mattress for people with low back pain. Some people find that a small firm pillow between the knees when sleeping on the side helps to ease symptoms at night.
Degenerative disc disease . This is not really a disease but a term used to describe the normal changes that occur in your spinal discs as you age. Over time, the discs in your spine break down, or degenerate. A loss of fluid in the discs or tiny tears or cracks in the outer layer of the discs can occur over time. The breakdown of the discs can cause back or neck pain, arthritis, spinal stenosis, or a herniated disc. A sudden injury to your back, such as from a fall or a car crash, may also start this process. This problem can occur anywhere on your spine. But in most cases, it occurs in the discs in the lower back and neck.

Radicular pain. This type of pain can occur if a spinal nerve root becomes impinged or inflamed. Radicular pain may follow a nerve root pattern or dermatome down into the buttock and/or leg. Its specific sensation is sharp, electric, burning-type pain and can be associated with numbness or weakness (sciatica). It is typically felt on only one side of the body.
